Nephrology

Neurology
LOCATION: Main Hospital, First Floor, across from pharmacy

The Neurology Clinic provides neurological assessment and testing for patients with neurological diseases or injuries of the brain, spinal cord, and peripheral nerves and muscles. Referrals are required to be seen in Neurology clinic.
All neurological emergencies must report to the emergency room for acute treatment.
Examples of neurological emergencies include:
Unusual or sudden onset of confusion, dizziness, loss of balance, loss of bowel and/or bladder control, weakness, facial droop, slurred speech, severe headache, fainting, unconsciousness, and/or acute injury.
OB/GYN Clinic
LOCATION: OB-GYN Services Flight is located through the clinic main entrance of the facility on second floor, above Family Health and across from Force Health.
Hours of Operation: Monday-Friday, 7:30 a.m. - 4 p.m., closed on federal holidays, base down days and readiness training days (first Wednesday of each month, unless designated differently due to mission).
New OB patient records establishment 8 - 11 a.m. and 1 - 3 p.m.
Our clinic staff consists of OB/GYN physicians, Certified Nurse Midwives, Nurse Practitioners, residents, registered nurses, medical technicians, and administrative personnel.
Occupational Therapy
LOCATION: Take first street past hospital, you will pass Riptide Inn and two other medical buildings on left, clinic is co-located in next building with Physical Therapy on the left.

Hours of Operation: Monday - Friday, 7 a.m. - 4 p.m.
Patient must be referred by their Primary Care Manager. No walk-in service available.
**If patient had surgery performed by off base provider, please bring operation report and any other pertinent records to your initial appointment.
Oncology/Hematology Clinic
LOCATION: 1st Floor, across from Outpatient Pharmacy

Hours of Operation: Monday - Friday, 7:30 a.m. - 4:30 p.m.
The Oncology/Hematology clinic specializes in treatment of blood diseases and disorders as well as cancer-linked diseases. Referrals are required.
Ophthalmology Clinic
LOCATION: Enter through Specialty Clinic entrance, take escalator to second floor, and turn right
Hours of Operation: Monday - Friday, 7 a.m. - 4 p.m.
We close the first Wednesday of every month.
Ophthalmology is currently seeing active duty service members, retirees and dependants. All patients require a referral from their PCM or off base provider prior to been seen at ophthalmology clinic.
Patients should arrive 15 minutes early for their appointments; bring a current list of their medication, current pair of glasses and or contacts. Patients are encouraged to schedule an annual-biannual eye exam or diabetic exam.
Optometry Clinic
LOCATION: Outpatient Clinic Tower, second floor, take the escalator from the pharmacy area.
PHONE: 883-8611, then choose prompt 4 to speak to an optometry staff member.
Hours of Operation: Monday - Thursday, 7:30 a.m. - 3 p.m.; Friday: 7:30 a.m. - noon
The clinic is closed the first Wednesday afternoon of each month for medical readiness training, unless designated due to mission.
Appointments: 883-8600 Speciality clinic option
Appointments are offered to active duty members enrolled to the 96th MDG. At this time, optometry appointments cannot be booked through Tricare OnLine (TOL). Appointments for AD dependents and retirees are offered on a space-A basis only as manning and mission requirements allow.
TRICARE Prime members may schedule an appointment with any TRICARE approved provider.
Most patients will receive an updated contact lens prescription, provided they wear their contact lenses to the appointment and provide their contact lens data to the clinic during their appointment.
To ensure maximum benefit from your appointment, please bring the following items to all exams:
1. ID card
2. Current glasses and sunglasses
3. Most recent contact lens prescription
4. Contact lens boxes or packages

Pain Management - Interventional Clinic
LOCATION: Co-located in Pediatric Modular building, in front parking area of Inpatient Tower entrance

Hours of Operation: Monday - Friday, 7a.m. - 4 p.m.
Clinic offers multi-modal and multidisciplinary care for patients that have been diagnosed with chronic pain. Patients must be referred by their PCM. The clinic is interventional with procedures ranging from conservative therapy such as acupuncture, joint injections, and muscle treatments to invasive therapies such as spinal injections, minimally invasive spine surgery and implantation of complex neurostimulators and opioid pumps. Interventions are tailored to fit the patient's pain needs. Patients must arrive at appointments 30 minutes prior to scheduled appointment time to complete necessary intake paperwork.
Pediatrics
LOCATION: In modular building in parking lot in front of "Hospital Entrance" door
Hours of Operation: Monday, Tuesday, Thursday and Friday, 7 a.m. -
4 p.m; Wednesday, 8 a.m. - 4 p.m.
The clinic closes the first Wednesday of month at noon for medical readiness training, unless designated due to mission.

Pharmacy Flight
LOCATION: The Main Pharmacy is located on the first floor of the hospital between the Immunizations Clinic and the Laboratory. The Satellite Pharmacy is located in the Base Exchange parking lot, next to the Class 6 store.

Hours of Operation: The Hospital Pharmacy is open Monday - Friday from 7:15 a.m. - 5 p.m. The Satellite Pharmacy is open Monday - Friday from 9 a.m. - 5 p.m. with drive-thru service until 5:30 p.m. Both pharmacies are closed on Federal holidays and base down days. The Satellite Pharmacy will not open until 9 a.m. on the first Wednesday of each month for readiness training, unless designated differently due to mission.
Physical Therapy

If patient has an acute injury acquired two weeks or less, they may walk-in without a referral. Walk-in hours are from 8 - 9 a.m. in the Physical Therapy clinic and in the Family Health Clinic.

Public Health/Force Health Management
LOCATION: Second floor/clinic addition

Hours of Operation: Monday - Friday, 7 a.m. - 4 p.m.
The clinic closes at noon the first Wednesdays of the month for training, unless designated due to mission.
Occupational Health:
Occupational Health Exams may scheduled by calling Central Appointments at 883-8600 and navigating options to schedule with Flight Medicine.
BIO Respirator Fit Testing:
LOCATION - Bldg. 635 (not within the hospital)

Respirator Fit Tests are done on Fridays from 9 a.m. - 3 p.m. Walk- ins are welcome.
Gas Mask Fit Tests are on Tuesdays from 8 a.m. - 3 p.m. by appointment only. Please contact your Unit Deployment Manager for all gas mask appointments.
Audiograms (Hearing Tests) may walk in any duty day during clinic hours from 7:30 a.m. - 3 p.m., or call central appointments at 883-8600 and navigating options to schedule an appointment with Force Health Management/Public Health.
Pulmonary Clinic
LOCATION: First floor, directly across from the pharmacy

Hours of Operation: 7 a.m. - 4 p.m., Monday through Friday. We are closed on federal holidays and base down days. We are also closed for training from 12 - 4 p.m. on the first Wednesday and the third Thursday of each month, unless designated differently due to mission.

Surgery Clinic
LOCATION: Temporary Phasing Facility - TPF #2 (modular building) across from Tricare building

Hours of Operation: Monday - Friday, 7 a.m. - 4 p.m., Monday - Friday. Closes at noon the first Wednesday of every month for readiness training, unless designated differently due to mission.
Pre-op instructions for adults & children
The General Surgery clinic provides a complete line of surgical and sub-specialty care to include General, Thoracic, Vascular, Bariatric, Plastics and Reconstructive surgery. Care is available to all eligible beneficiaries. Initial access is by referral only.
Urology Clinic
LOCATION: Specialty Clinic entrance, first floor, directly across from the Radiology Check-In desk

Hours of Operation: Monday - Friday, 7 a.m. - 4 p.m.
Referrals are required to be seen in the urology department. These can be obtain through your Primary Care Manager.
Urology is a surgical specialty providing care for a wide variety of urological problems and disorders, i.e. Prostate problems, Prostate/Kidney/Bladder Cancer, Urinary Stones and Male Infertility. If you've been seen by 96th MDG in urology in the past five years you do not need a new referral.
X-RAY - Radiology
LOCATION: Specialty Clinic entrance, first floor

Hours of Operation: Monday - Friday, 7 a.m. - 4 p.m.
All radiology sections are closed on holidays and base down days.
Other services provided: Mammography, Computed Tomography (CT/CAT Scan), Film Library, Nuclear Medicine, Ultrasound and MRI.
Main Radiology (X-Ray) is the part of Radiology most people go to for imaging. This section is also called "X-Ray" because of the type of electromagnetic radiation used to produce the images of bones and soft tissues. We provide services to all the clinics in the hospital.
After hours (at night, on weekends and holidays) we have one technologist on duty to provide urgent services, primarily for the Emergency Room, Operating Room, and Inpatient wards.
