PATIENT RIGHTS

As our patient, you are entitled to safe, considerate, respectful & dignified care at all times.
Access Emergency Services
You have the right to:
- Receive emergency care without preauthorization where & when acute symptoms are so severe that a "sensible layperson" would want emergency care to prevent serious harm or death.
Respect & Nondiscrimination
You have the right to:
- Timely considerate, & respectful care at all times and under all circumstances
- An environment of shared respect which is free from discrimination
- Care that is considerate and respectful of your personal values and beliefs
- Access to Pastoral care and other spiritual services
- Access to effective communication, including hearing & speech impaired services and translation services
You are responsible to:
- Have respect for the rights of other patients & Military Treatment Facility (MTF) Staff
- Ensuring that your guests also follow hospital rules & regulations
Choice of Providers & Plans
You have the right to:
- Accurate information about the TRICARE program to include covered health benefits & health plan options
- A choice of healthcare providers within TRICARE Prime network or the MTF, within constraints of MTF policies.
You are responsible to:
- Become educated about the TRICARE coverage you have selected, options, & rules, to include required fees
Participation in Your Care & Care Decisions
You have the right to:
- Receive accurate, assessable & understandable healthcare information
- Ask questions & receive timely answers to those questions
- Choose someone to make decisions on your behalf if you cannot do so
- Request an individual of your choosing to be present for your emotional support during your stay in the hospital unless the individual's presence infringes on others' rights, safety, or is medically or therapeutically contraindicated.
- Know all of your treatment options including the option of no treatment
- Know the risks, benefits and consequences to each of these options
- Refuse care as allowed by law
- Know the name of the physician or other practitioner who has primary responsibility for your care
- Know what areas of the hospital do & do not honor advance directives
- Receive appropriate pain assessment & pain management
- Participate in ethical questions that arise in the course of your care, including issues of conflict resolution, withholding resuscitative services, forgoing or withdrawal of life-sustaining treatment, & participation in investigational studies or clinical trials
You are responsible to:
- Tell your healthcare professional everything you know about your illness or condition, past health care and any & all medications or supplements you are taking
- Inform the doctor or nurse about any changes in how you feel
- Participate in decisions related to your healthcare & ask questions if you do not understand any part of the proposed treatment
- Follow your provider's plan of care
- Maximize healthy habits such as exercise, not smoke, eat a healthy diet, & not knowingly spread disease
- Accept the consequences if you refuse to follow the plan of care
- Follow hospital rules & regulations regarding your care & conduct
- Keep appointments or call to change or cancel them in advance
Confidentiality of Health Information
You have the right to:
- Privacy & protection of your personal health information
- Communicate with healthcare providers in confidence as allowed by law & regulation
- Review & obtain a copy of your own medical records
- Request amendments to your records, within specific legal limits
Appeals & Complaints
You have the right to:
- A fair & efficient process to appeal medical necessity decisions by your MTF or by TRICARE that includes both internal review & independent external review
- To discuss complaints with your providers or Patient Relations rep
- Elevate concerns regarding patient care & safety to the Patient Relations Coordinator/Hospital Management Representative or The Joint Commission via email at complaint@jointcommission.org or fax to: 630-792-5636.
- Contact a TRICARE Debt Collection Assistance Officer if unable to pay fees
You are responsible to:
- Report wrongdoing & fraud to appropriate authorities
Information Disclosure
You have the right to:
- Review a provider directory maintained in TRICARE office to assist in understanding your provider's education & training and selecting a Primary Care Manager
- Review publicly-reported MTF quality measures at www.jointcommision.org
You are encouraged to ask questions about any of these rights that you do not understand. DoD Instruction 6000.14, Patient Bill of Rights & Responsibilities in the Military Health System, September 26, 2011, completely describes your rights & responsibilities and a copy is available from the MDG staff, Patient Relations Representative or Coordinator.
