EGLIN AIR FORCE BASE, Fla. -- Dependents and children age 12-15 with base access can now receive the Pfizer COVID-19 vaccination on or off base.
All COVID-19 vaccinations (for all ages) are by appointment only through the 96th Medical Group’s Allergy and Immunizations Clinic. Appointments are made via the Defense Health Agency’s vaccination appointment portal.
Those people coming in for appointments should bring a completed DHA Form 207 if possible.
