The 96th Medical Group will provide the H1N1 vaccine to active duty military and DOD civilians Jan. 19 - 22.

The vaccine will be given from 6 a.m. to 6 p.m. at the 9th Special Operations Squadron auditorium.  The vaccine is mandatory for active duty members.

Those receiving the shot should fill out and print the H1N1 screening form and bring it with them to the auditorium.

Any active duty who've received the H1N1 vaccine at a civilian facility must provide proof of immunization to the auditorium to have their status updated.

Seasonal and H1N1 are currently available to specific target groups at the Immunization clinic during normal duty hours.  For more information, please click here
